Our patients have to deal with a myriad of pain and dysfunction issues that out of necessity become chronic as the patient has to go through the everyday necessities of life. The patient is forced to live with these symptoms and tries to mitigate with drugs, continuous band aid treatment modalities and in many cases significant irreversible treatment that may not even address the issue at hand. As contemporary dentists, we have the ability to interact with our patients in a manner that allows them to inform us of such chronic issues that they may be facing and at the same time be able to understand the presented physiology by the patient to render treatment that allows for cessation of such pain and dysfunction.
In performing long lasting comprehensive contemporary dentistry, Form, Function, Esthetics and Stability are the attributes that we strive for and should for good reason. In the end, it is "physiology" that informs and governs the long term end result. In generalities, anatomy (i.e. Form) gives us guidance whereas physiology determines the end result. As Sir Charles Sherrington proved with a resulting Nobel Prize, that "Physiological Economy" is at the center of a person's well-being.
Dr. Kherani will discuss the theory and science behind the Neuro Muscular Occlusion philosophy and its application in treating numerous cases that range from aesthetic needs to pain management in a contemporary setting. A sound diagnosis followed by a physiologic treatment modality will prevent both overtreatment and under treatment of the patient leading to a permanent and stable cure of the conditions presented.
The much ignored arena of Sleep Disordered Breathing will also be considered in the overall diagnosis leading to treatment that allows the alleviation of this harmful disorder with far reaching complications that can result in increased mortality and morbidity. Surgical decision making in aesthetic implant dentistry
The dental rehabilitation in the esthetic zone is often clinically challenging. The goal of esthetic implant therapy is to provide a predictable and esthetically and functionally satisfying treatment outcome with a low risk of esthetic complications. It is desirable to achieve this goal with a minimum trauma and number of surgical procedures and in the shortest possible treatment time from tooth extraction to implant restorations.
The aim of this presentation is to introduce a surgical decision making to help the clinician in the selection of the most adequate surgical technique in the implant treatment of esthetic areas in order to reach the objectives described above. Each surgical decision is presented with a case report. In addition, the biologic rationale is discussed.

Local Anesthetic Performance; Fiction, Fact and Advancements (Precision Buffering)
This presentation will focus on the science and clinical performance of local anesthesia, including recent advancements. A review of the documented studies on the time course for soft tissue analgesia and pulpal analgesia will lend to the discussion on the commonly held beliefs under which many dentists organize their practice day.
The presentation will discuss the role that alkalinization (pH buffering) standard dental anesthetic cartridges immediately before delivering the injection can affect the anesthetic's performance, the patient experience, and the practice's schedule. The presentation will describe and demonstrate a novel ex vivo precision cartridge buffering system.

Innovative and new instruments to preserve bone during extractions
The need to preserve as much bone as possible is crucial to the predictability of successful implant osseous integration. With the tremendous popularity of implants by dentists and their acceptance by society, replacing edentulous areas with predictability will provide you with the key to success.
This presentation presents and discusses new and innovative instruments that will effectively aid you in the regeneration of predictable bone and its preservation during extractions of teeth. Classical extractions have all too often led to resorbsion of the buccal bone of the extracted area. By their putting pressure on the buccal bone, the bone resorbs, leading to loss of this buccal bone. Consequentially, implants, which need to be placed in adequate supportive bone, are too often placed too lingually, where the majority of the bone is preserved. This leads to the prosthetic coverage of the implant having occlusal as well as embrasure compromises.
These new instruments change the direction of the pressure of the extraction, avoiding pressure towards the buccal or lingual bone. By changing this direction, these new instrument avoid the probable resorbsion, preserve bone, and promote successful regeneration of a wider B-L osseous height and ridge. The implant will then be ready to be placed in a more desirable position, both physiologically and occlusally.
